- School, College, or University. You must be a currently enrolled student at a recognized accredited institution.
- Your major.
- Your minor. Leave blank if none.
- Your year in school (Sophomore, Junior or Senior) at the time of application.
- Overall Grade Point Average (GPA). Please use the 4.0 scale. Include only the courses that you have completed.
- Biology, Chemistry, Physics, Math (BCPM) GPA. Please use the 4.0 scale. Include only the science courses, including physiology if applicable, that you have completed.
A. Personal Narrative or Statement - Your personal narrative or statement should address the following:
- Your relevant background - academic, personal, and service
- Your career goals, i.e. your interest in pursuing graduate or professional-level work in a specific discipline/area
- The science questions that interest you most and the reason(s) for your interest(s).
- The specific areas in which you would like to conduct research and the factors that influenced the development of those interests.
- Your interest in participating in UPRIME at the University of Minnesota.

B. Adversity Statement
Please describe a challenge you have faced while you were in college. Include how you have overcome or are overcoming this challenge.

A. Choose up to three research areas of interest. You must indicate at least one.

B. Previous Research Experience – Provide a brief description of your previous research experience(s). If possible, describe the project, your contribution to the project, and tasks performed.
If you do not have any previous research experience, then describe what laboratory course work you have taken.

C. You are tasked with designing an experiment with a drug or treatment. Briefly describe your design for this experiment.
Your answer should include:
• At least one technique (For example: PCR, Western blot, microscopy, biochemical assay) that you would use in your experiment.
• Appropriate controls.
• Predicted results

- Letter of Recommendation: You must provide the names and email addresses of two recommenders. One of them can be your research advisor.
- Signature: By signing and submitting this application you certify that the information you have provided in this application is truthful and to the best of your knowledge.
